NHS England\u2019s<\/a> Dementia Team have just published their new guide My Future Wishes: Advance Care Planning (ACP) for people with dementia in all care settings<\/strong>.<\/a><\/em><\/p>\n

NHS England developed the guide, in response to the \u00a0Dementia Challenge 2020<\/i><\/a>\u00a0which maintains that “By 2020 we would wish to see\u2026 All people with a diagnosis of dementia being given the opportunity for advance care planning.<\/em>”\u00a0In the view of NHS England the ACP is “fundamental for everyone living with dementia. It enhances choice, aids delivery of person-centred end of life care, helps to guide care when mental capacity is lost and provides support for families and carers<\/em>.”<\/p>\n
